CHNC’s QI collaborative, PROJECT HOME, aims to improve the percentage of infants receiving human milk at discharge.

Collaborative sites have seen improvements in the following processes:

• Documented needs assessment within 72 hours (increased from 79.19 to 86.09%)
• Weekly multi-disciplinary rounds related to human milk (increased from 70.19 to 78.79%)
• Oral care (increased from 54.99 to 62.18%)
• Physician/APP education within 72 hours (increased from 30.37 to 50.38%)
• Skin-to-skin kangaroo care (increased from 44.64 to 49.02%)
